I understand that use of Alpha Fitness facilities, equipment, classes, and training services involves inherent risks, including but not limited to serious bodily injury, permanent disability, or death. I voluntarily assume all risks associated with participating in gym activities, including use of weights, machines, cardio equipment, group exercise, and any other services offered by Alpha Fitness. To the fullest extent allowed by Kansas law, I release and hold harmless Alpha Fitness, its owners, employees, contractors, trainers, and agents from claims for injury, loss, or damage arising out of my use of the facility, except to the extent caused by gross negligence or willful misconduct. I certify that I am physically able to participate, and I agree to stop activity immediately if I experience pain, dizziness, or any other concerning symptoms.

I authorize Alpha Fitness to charge the payment method I provide for my membership dues, enrollment fees, recurring renewals, approved add-ons, replacement access devices, and any other charges I authorize in connection with my account. I understand that recurring membership charges will continue according to my selected billing cycle until my membership is cancelled under the terms of my agreement. I agree to keep a valid payment method on file and to update my card information promptly if it changes or expires. I understand that failed charges may result in my account becoming past due, suspension of gym access, and additional collection activity allowed by my membership agreement.

I grant Alpha Fitness permission to use photographs, video, or digital media that may include my image for internal use, promotional materials, website content, and social media marketing. I understand that no compensation is owed for such use. I may revoke this consent in writing for future use, but I understand that previously published materials may not be retractable.
