GENERAL GYM RULES & CONDUCT Members agree to: Use equipment properly and safely. Wipe down equipment after use. Stop exercise if feeling pain, dizziness, or discomfort. Wear appropriate gym attire. Treat staff and members respectfully. Members & 24 hour pass members are not allowed behind counter desk area, inside kitchen area, inside break room area or in the Hair Salon area when business is closed or after hours. FINANCIAL COMMITMENTS AND OBLIGATIONS Billing is automatic. No refunds for partial billing periods. Failed or declined payments may result in restricted access until resolved. A late fee may apply after 3 days. Founders memberships remain valid for life only while active. If canceled, the rate is forfeit. Membership cancelations are required to pay remaining membership balance. ACCESS DEVICE RESPONSIBILITY I agree that my key fob / access code is for my use only. I will not allow non-members to enter. Lost devices must be reported immediately. Unauthorized entry may result in termination of membership. EQUIPMENT USE I agree to use equipment properly and safely, return equipment after use, clean equipment after use, report damaged or unsafe equipment, and follow all posted rules. PROHIBITED CONDUCT During staffed or unstaffed hours, I will not bring guests without authorization, use equipment improperly, engage in unsafe or illegal activity, or tamper with security systems. MEMBERSHIP TERMINATION Violation of gym policies may result in immediate suspension or termination of access without refund.

ADULT & MINOR (AGES 13 - 18) LIABILITY WAIVER I understand that fitness activities involve inherent risks including muscle strain, heart complications, injury or death. I voluntarily assume all risks and release Alpha Fitness, its owners, employees, trainers, contractors, and affiliates from any and all claims, injuries, damages, or liabilities including those arising from negligence, to the fullest extent permitted under Kansas law. I confirm that I or my child are physically capable of participating in exercise activities, have no medical conditions that would prevent safe participation, or I have consulted a physician. I understand that I am responsible for all my personal belongings and release Alpha Fitness, its owners, employees, trainers and affiliates from any and all claims, damages or liabilities to the fullest extent permitted under Kansas law. EMERGENCY MEDICAL AUTHORIZATION I authorize Alpha Fitness as the care giver to obtain emergency medical care for myself and/or my child at my expense. ACKNOWLEDGMENT & SIGNATURE By signing below, I confirm I have read, understand, and agree to all terms.

24-Hour Gym Access Agreement & Liability Waiver 1. Acknowledgment of Unstaffed Hours I understand that this facility operates with unstaffed hours, during which no employees or trainers may be present. I accept full responsibility for my safety and conduct while using the gym during these times. 2. Assumption of Risk I understand that the use of exercise equipment and participation in physical activity involves risks, including but not limited to: Injury from equipment or improper use Slips, falls, or accidents Medical emergencies Injuries while training alone I voluntarily assume all risks, known and unknown, associated with using this facility at any time. 3. Release of Liability I release, waive, and discharge Alpha Fitness Gym, its owners, employees, and affiliates from any and all liability for injury, illness, death, loss, or damage resulting from my use of the facility, including during unstaffed hours. 4. Medical Responsibility I certify that I am physically able to participate in exercise. I understand that the gym does not provide medical supervision and I am responsible for my own health and emergency response. 5. Emergency Procedures I understand emergency contact numbers and procedures are posted in the facility, I should call 911 in case of emergency, and first aid equipment locations are posted. 6. Security & Surveillance I understand the facility may use security cameras for safety and monitoring. My entry and activity may be recorded. 7. Indemnification I agree to indemnify and hold harmless the gym from any claims caused by my actions, negligence, or misuse of the facility. 8. Governing Law This agreement shall be governed by the laws of the State of Kansas. 9. Agreement I have read and fully understand this addendum. I sign voluntarily and agree to all terms.

I grant Alpha Fitness permission to use photographs, video, or digital media that may include my image for internal use, promotional materials, website content, and social media marketing. I understand that no compensation is owed for such use. I may revoke this consent in writing for future use, but I understand that previously published materials may not be retractable.
